In patients on therapy with modern GLP-1 agonists for weight loss, including Semaglutide (Wegovy) and Tirzepatide (Zepbound), therapeutic responses are not identical across patients. Individual differences in metabolism, lifestyle, and drug tolerance can affect how quickly the effects will manifest.

According to obesity subspecialist W. Scott Butsch, these weekly injections reach their peak activity three days after administration. Clinical effects can become noticeable as early as the first days of therapy (most often in the form of reduced appetite), but gastrointestinal side effects, such as nausea, vomiting, or diarrhea, can also occur during that period.

These two preparations belong to the broader therapeutic class of GLP-1 agonists, drugs that mimic the action of glucagon-like peptide-1 (GLP-1) – a hormone produced in the intestines and central nervous system of mammals.

Its physiological role is multifaceted: it regulates insulin secretion and thereby contributes to glycemic control, while also participating in mechanisms of appetite regulation and energy balance.

By acting on specific brain centers associated with feelings of hunger and satiety, GLP-1 agonists can reduce appetite. In addition, they slow stomach emptying, prolonging satiety and potentially reducing total caloric intake.

However, as obesity subspecialist W. Scott Butsch points out, the effects on appetite may not be noticeable immediately after the first application; in some cases, it may take several weeks and several consecutive doses to achieve a full therapeutic response. “With some drugs, it takes time and repeated administration to develop the maximum effect.” He emphasizes.

Factors that can Influence How Long it Takes to Work

The response to GLP-1 agonist therapy varies among patients. The dynamic of the effects may depend on several individual and lifestyle factors, including:

In some cases, it is necessary to increase the dose gradually until the therapeutic effects become clear.

That is why it is crucial to approach therapy patiently and with realistic expectations. The titration process can last several months, and only after reaching the maximum optimal dose can the full benefits be observed.

In Dr. Butsch’s experience, a lack of response to therapy can be frustrating, but it does not necessarily mean the drug will not show efficacy after adequate dosing.

There is no clear timeline for when the first results of GLP-1 agonist therapy can be expected, as each person reacts differently.

“Clinical studies show that there is a big difference between the results of patients, both in the amount of body weight loss, and in changes in appetite, but also in the occurrence of side effects,” points out obesity subspecialist W. Scott Butsch.

“That’s why it’s hard to predict what benefits an individual will have and when they’ll first notice them.”

However, he notes that research shows early response to GLP-1 agonists can be an indicator of long-term benefits, helping plan and adjust therapy for the best results.

“According to obesity subspecialist W/ Scott Butsch, patients who lose more than 5% of their body weight in the first three to four months of therapy are more likely to maintain that weight after a year.”

“Similarly, a recent study with Tirzepatide shows that people who lose 10-15% of their body weight in the first six months are more likely to keep the weight off after 12 months.

Bottom Line?

Although future research will explain how quickly GLP-1 agonists show an effect, Dr. Butsch emphasizes that there is already knowledge that can help the drug work optimally.

“The key is to continue making lifestyle changes, whether it’s improving sleep, eating healthier, or getting regular physical activity, while you and your doctor evaluate whether this therapy is right for you,” he explains.

“This process can take several months, as drug doses are gradually titrated until the optimal effect is reached.
